What Is Needed for a Name Change?
Have you already decided that you wish to change your name? Filing for a name change is easiest when it is included in the terms of your divorce. There should be some paperwork included in the filing forms, with an option to choose your name to be changed.
You would then show the courts the following documents:
- Identification (driver’s license, state ID, or passport)
- Proof of your date of birth (birth certificate, hospital records, adoption records)
- A certified copy of your divorce papers
Your divorce decree will act as the official, legal proof that you have requested a name change. If you did not include this in your divorce decree, then your other option is to file a form with the California County Clerk’s office. The next steps would be to notify the Social Security Office as well as the Department of Motor Vehicles that you have made this name change.
